The discipline of health care risk management has much to offer in the way of helping health care providers understand how to avoid errors and the patient injuries and litigation that may result. The purpose of this course is to supply health care providers with a basic understanding of the importance of accurate, timely and thorough documentation for patient care and for risk management. This course will discuss how the medical record fulfills three important functions. First, it provides information about the patient's current condition, treatment response, and progress. Second, it serves as a legal record of the care provided. Third, it provides information used for reimbursement by third party payers.

COURSE OUTCOMES -- Upon completion of this course, you will be able to:
- Explain the three main purposes of documentation.
- State documentation guidelines that relate to being: factual; accurate; complete; and timely.
- Describe what is required to maintain patient confidentiality and list several exceptions to confidentiality rules.

Intended Audience: This activity was developed for physicians, nurses, and other healthcare professionals.
